Advance Directives
Because we value a patient's right to make decisions, and we encourage patients to actively participate in their care, we are providing this information to explain your right to make healthcare decisions, and how to plan for those decisions to continue if and when you are unable to speak for yourself. Various federal and state laws also govern certain requirements for the communication of this information, all with the intent of allowing you to maximize the control over medical decision making. If you would like to plan for the future of your healthcare choices, an Advanced Directive form is available in our Admitting Department and can be completed and brought to Medical Records to be scanned into your health record for future reference.
