Health Information Management
The Health Information Management Department (Medical Records) provides and protects Protected Health Information in a confidential manner. We strive to produce quality information. The "HIM" staff interprets diagnoses and procedures into codes for payment to healthcare providers. Dictated reports are transcribed. We strive to work closely with physicians, hospital staff, patients and other customers to promote the values and the mission of Crittenden Community Hospital.
The HIM Department takes confidentiality and Security of Health Information very seriously. The requirements for Release of Information are always to bring picture identification with you to obtain a copy of your medical records. You must sign a release of information to obtain or release your records. No one else can sign a release for your records except you, including your spouse. In a divorce case, proof of custody must be presented to obtain records on minor children. Also, legal documentation should be presented to obtain records on a deceased patient.
HIPAA Privacy: When placing a privacy complaint, this must be done in writing. Please contact the hospital to obtain a copy of the form.
