Home Health Registered Nurse
Crittenden Community Hospital has an immediate opening for Home Health Registered Nurse, Full-time Day Shift.
JOB SUMMARY:
The purpose of the licensed nurse is to provide skilled nursing, including skilled observation, to the homebound patient. Skilled assessments on home health referrals will be done. Population will include caring for prenatal, neonatal, pediatric, adolescent, adult and geriatric patients.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1.Provide services in accordance with agency policies. Carry out procedures within their license guidelines
2.Prepares progress notes and shows evidence of good observation, reporting, and recording skills. Completes all required paperwork in a timely matter.
3.Maintain good communication skills
4.Incorporate a verbal medical order into the plan of treatment and prepare to send the form to secure the physicians countersignature according to agency policy
5.Complete paper work as required by all Medicare Conditions of Participation.
6.Assists the patient in learning self-care techniques, nutritional information, about medication usage and about physical condition, while also assessing needs of patient for health teaching and reporting such findings
7.Assess the patient's emotional and physical needs, and implements care specific to such need, making additions to the plan of treatment after such evaluations
8.Performs other related duties as assigned
10.Promotes and participates in performance improvement activities

13.Must attend all mandatory in services
14.Complies with all applicable state and federal laws and conducts job activities in an
ethical manner as outlined in Crittenden Community Hospital policies and procedures and in accordance with the standards and practices stipulated in the Corp. Compliance Plan.

QUALIFICATIONS:

Education, Training or Experience:
Current state licensure
Maintain 15 continuing education hours annually for license renewal
Must have working automobile available for use on daily basis, responsible for maintenance on vehicle
Responsible for providing proof of insurance coverage upon request
Previous Home Health experience preferred,
CPR certified. If not certified, must be certified within 3 months of hire date.
